If there’s any practical effects in this scene, is the actor trying to get a hard on while a squeeze bottle of blood and brain is being hidden somewhere around his head? Meanwhile, Hagan is actually getting penetrated while performing stage combat?

I just don’t see it.

I can, however, imagine them in the editing bay noticing that you can see straight through and adding something in the shadows.
